A Name; A Title; It's All the Same . . .

In many cultures and esp. in Asian ones, calling people by their titles (Directer; Elder Sis; President; Second Sister; etc.) is very common. Since this is so, when writing these titles in place of a given name, capitalization is called for.

Therefore, when used as a description, caps are not needed: Everyone, this is my eldest sister . . . But, when it is their title Everyone, Eldest Sister (Jade)

Look! The president is coming!! Mr. President! The 1st time uncapitalized, 2nd is used as a name, so it is capitalized.

Also, in writing a book, movie, article title, they are usually capitalized except for words such as: 'a/'an'/ 'and' for 'the', it is common to have the first 'the' in caps and any other in lower case. "The Young Boy Ran Up the Hill"; "How Does the Product Make Your Hair Shine?"
