"Flames of Desire" is Ulsan MBC's (a branch of Munhwa Broadcasting ) biggest project and features a lot of scenes shot in Ulsan.
1) What ever happened to Kim Nam Joo , Shin Se Kyung and Park Shin Hye's casting ?
Originally casted as the lead in this drama, Kim Nam Joo had to drop out due to the upcoming Queen of Reversals, Shin Se Kyung had quite a schedule (rumor) and Park Shin Hye's casting was not confirmed.
2) Why has the drama been renamed from 'A Woman will only love once in a lifetime' to 'Flames of Desire' ?
There's a lot to this question, first of all, it appears the script had changed drastically 3 months prior to a press release about new casting. A lot of people speculate that it went from "light and breezy" rom-comedy [because of the timeslot-family timeslot] to Makjang (immoral) Ulsan MBC had gathered resources for a big project , hence they picked a darker script.

She's a respected screenwriter and most prominently known for MBC's 2008 'La Dolce Vita'
4) Where were the US scenes shot?
Ironically, they were shot in S.Korea in English village (an English immersion touristic area)
