"비주얼 깡패" is literally translated to “visual gangster” and it probably originates from the expression "어깨 깡패" ("shoulder bully" or "shoulder gangster"), which refers to people who have very broad shoulders (usually viewed positively). So "비주얼 깡패" is just used to describe someone who is very good-looking. And "중2병" is literally translated to “2nd year in middle school disease,” which is the emotional changes that one might go through during puberty or adolescence.
